If you have questions or concerns you may call us directly at 801-792-3635. Please allow 24 business hours to respond  to your voicemail message if we are not available at that time. 
If you cancel your visit at any time prior to your scheduled medical visit a full refund will be applied to your credit card. Otherwise you are responsible for the full cost of the visit. However, we will consider extenuating circumstances on a case by case situation.
If we prescribe medication and send it to your pharmacy, and they ship it to you, please reach out to us and the pharmacy if there are any issues. We do not pay for shipping of any medications.
If you need to dispute a charge, or wish to request a dispute by your credit card company, please notify us first. We will check to see if any duplicate charges have been made by mistake and if so refund you the full amount of the duplicate charge. You also have the right to contact your credit card company or bank to confirm duplicate charges. If, for some other reason you wish to dispute a charge we will make every effort to make things right for you.
We do not complete prior authorization forms for any reason or for any medication. You are welcome to see a primary care provider who takes your insurance for those needs.
